1. Copy of your current registration or the letter you received from Secretary of State regarding the renewal of your registration
2. Total miles in every state you traveled for the period July 1, 2018 through June 30, 2019 (IFTA reports for 3rd and 4th quarter of 2018 and 1st and 2nd quarter of 2019 or trip reports, statements, log books for the period requested)
3. HWY Use Tax (Form 2290) for all vehicles that you are renewing
- HWY Use Taxes currently acceptable are for the period 2018-2019
- IRS stamp or e-file logo must be clearly visible
